Post Acute Care

At Orlando Internal Medicine, we take pride in providing comprehensive care to our patients across the healthcare continuum.

We work with Long Term Acute Care hospitals, Inpatient Rehabilitation Facilities, Skilled Nursing Facilities and Long Term Care Facilities. Our Acute care team will help navigate the appropriate Post-Acute settings for patients where we have dedicated teams of Physicians, Nurse Practitioners and Physician Assistants who will provide a smooth transition for patients and maintain continuity of care in multiple Post-Acute facilities across Central Florida.

Our highly trained team provides initial evaluations and follows patients throughout their stay. From medication reconciliation and management to coordination of specialty care our teams work to improve patient conditions and prepare them to return home.
